Meet our Dentist
Jeremiah J. Frasier, D.D.S
Dr. Frasier is a native of Muncie, Indiana and attended Ball State University. He graduated from Indiana University School of Dentistry in 2002. He then completed a hospital dentistry residency at the University of Washington in Seattle in 2004. There he received extensive training in pediatric dentistry, oral surgery, sedation dentistry, trauma management, and dental care for the medically compromised.
Dr. Frasier became the owner of Forest Creek Family Dental Care in 2011, but the southside Indianapolis dental practice has been serving the area for more than 20 years. Forest Creek Family Dental Care is a family and cosmetic dental practice serving Indianapolis, Greenwood, and surrounding areas.

Dr. Frasier and his wife, Susan, are proud parents of two children - Lucy and Simon. They enjoy going boating and camping in the summer, spending time with family, and attending their children's various activities.
Meet our Staff
Sunny - Office Manager
Sunny has been with Forest Creek Family Dental Care since 2004. She started as Dr. Frasier’s assistant and is now the office manager. Sunny graduated from Roncalli High School in 2001 and attended PCI for Dental Assisting and got her expanded functions certificate at IU School of Dentistry. Sunny enjoys helping in all aspects of the dental office. She is always there to greet every patient with a warm welcome and smiling face and help with any concerns or questions they might have.
Sunny resides on the south side of Indianapolis with her husband and kids. In her free time she enjoys spending time with family and friends, going camping, boating and lots of shopping.
Ashley - Front Office
Ashley has been with Forest Creek since 2020. She started her dental career as an Expanded Functions Dental Assistant in 2007. You will now see her in our front office greeting patients, scheduling appointments and answering any questions or concerns you may have.
Outside of work Ashley enjoys watching her three sons in various sports and activities with her husband Adam.
Heidi - Dental Assistant
Heidi joined our office in June of 2021. She attended Dental Office Training by Lynn for dental assisting and is certified in Dental Radiology. She is great at making every visit enjoyable and comfortable for our patients.
Heidi is expecting a baby boy, Ryder James, in August 2022. Outside of work, she enjoys spending time with her boyfriend Cody, nieces and nephew, and attending concerts with her friends.
Hannah - Dental Assistant
Hannah joined the staff of Forest Creek in 2017 as an Expanded Functions Dental Assistant and is certified in Dental Radiology. She enjoys dental assisting and making every visit pleasant for the patients.
Hannah is married to her husband, Nick. Together they have a beautiful daughter named Josie. Outside of work she enjoys being outdoors, eating out, spending time with family, and spoiling her dog Rufus.
Gina - Dental Hygienist
Gina has been with Forest Creek since May 2011. She started her dental career in 1996 as a Dental Assistant. She decided to go to hygiene school in 2005 graduating in 2007, and has been loving every moment!
Gina has a husband Marty and two teenagers Jonathan and Hannah , a dog Coco, and Frank the cat. Gina lived in Sudan with her parents and went to boarding school in Kenya for her junior and senior year. Her hobbies include, reading, hanging out with friends and being involved in her church.
Shannon - Dental Hygienist
Shannon joined Forest Creek in 2005. She began her career in dentistry as a dental assistant in orthodontics and later in radiology. She then obtained her degree in Dental Hygiene at Indiana University School of Dentistry in 2002.
Shannon lives on the south side of Indianapolis, is married and has 3 children and 5 grandchildren. In her free time, she likes to read, craft, travel and spend time with family and friends.
